Customer Setup

We are delighted that you are joining us as a new customer. In order that we may process the necessary documents to establish credit, payment terms and the other elements of your account, please complete this online form. You will need the following information handy to finish:
• General business contact information for your company and officers
• The contact information from 2 different businesses to serve as credit references
• Your banking information to complete an EFT Authorization
• A copy of your business financials for the past two years, with balance sheet included.
